Insurance Claims

As a courtesy to our patients, Ashland Surgery Center will bill your insurance for direct payment from your carrier if we are provided with the correct billing information. Please check with your insurance company to determine if our facility is a preferred provider of your plan. Your out-of-pocket responsibility will depend on your contract with your insurance company. Please contact them to determine your benefits. Ultimately, you, the patient, are responsible for charges incurred on the date of service.

Self-Pay Claims

If you do not have insurance or choose not to use your benefits, we require 100% payment of the estimated charges on the day of the procedure unless specified otherwise. All payment arrangements must be made with our billing department prior to your procedure.

Copay

If you have a copay, you will be asked to pay it at the time of your procedure.

Payments

If a balance remains on your account after your insurance responds, you will receive a statement in the mail. We accept cash, personal check, VISA, Master Card, Discover, American Express, and Care Credit.

Ashland Surgery Center (ASC) is owned by five local physicians: Dr. Chow, Dr. Ewing, Dr. Fredrickson, Dr. Schiedler, Dr. Welder, and Dr. Herron, who also perform procedures at ASC. If you have any questions or wish to have your procedure at another facility, please get in touch with your physician. Other physicians providing services at the center are independent providers of medical services and are not employees or agents of ASC. Questions about your treatment plan or charges billed by your surgeon or anesthesia provider should be directed to their offices. Thank you for choosing ASC.
