Insurance Claims

As a courtesy to our patients, Ashland Surgery Center will bill your insurance for direct payment from your carrier if we are provided with the correct billing information. Please check with your insurance company to determine if our facility is a preferred provider of your plan. Your out of pocket responsibility will depend on your contract with your insurance company. Please contact them to determine your benefits. Ultimately the patient is responsible for charges incurred on the date of service.

Self Pay Claims

If you do not have insurance, or choose not to use your benefits, we require 100% payment of the estimated charges on the day of the procedure unless specified otherwise. All payment arrangements must be made with our billing department prior to your procedure.

Copay

If you have a copay you will be asked to pay it at the time of your procedure.

Payments

If there is a balance remaining on your account after your insurance responds, you will receive a statement in the mail. We accept cash, personal check, VISA, Master Card, Discover, American Express and Care Credit.
